Biomechanical Comparison of Anterior and Direct Lateral Interbody Fusion with Supplemental Instrumentation

ثبت نشده
چکیده

INTRODUCTION: Direct lateral interbody fusion (DLIF) is less invasive than traditional methods, such as anterior lumbar interbody fusion (ALIF). While the direct lateral approach is safer for a variety of clinical reasons, immobilization of motion segments is crucial for complete bony union, regardless of approach. Therefore, biomechanical assessment is necessary to determine if the DLIF approach is a viable alternative to standard procedures, such as ALIF. The goal of this cadaveric biomechanical study was to compare the immediate postoperative stability of DLIF and ALIF. Further, any additional stiffness imparted by supplemental instrumentation was quantified. We hypothesized that the two approaches would be biomechanically equivalent, and that supplemental instrumentation would be comparably beneficial for both techniques.

Biomechanics of lateral plate and pedicle screw constructs in lumbar spines instrumented at two levels with laterally placed interbody cages.

BACKGROUND CONTEXT The lateral transpsoas approach to interbody fusion is gaining popularity because of its minimally invasive nature and resultant indirect neurologic decompression. The acute biomechanical stability of the lateral approach to interbody fusion is dependent on the type of supplemental internal fixation used. The two-hole lateral plate (LP) has been approved for clinical use for ...

Migrated XLIF cage: case report and discussion of surgical technique.

Extreme lateral interbody fusion (XLIF; NuVasive, Inc, San Diego, California) is a minimally invasive technique developed to avoid complications associated with traditional or minimally invasive anterior or posterior approaches to lumbar interbody fusion. It uses a direct lateral, retroperitoneal, transpsoas approach for placement of an interbody cage. To date, no reports of cage-related compli...

Biomechanics of posterior instrumentation in L1-L3 lateral interbody fusion: Pedicle screw rod construct vs. transfacet pedicle screws.

BACKGROUND The use of pedicle screws is the gold standard for supplemental posterior fixation in lateral interbody fusion. Information about the performance of transfacet pedicle screws compared to standard pedicle screws and rods in the upper lumbar spine with or without a lateral interbody fusion device in place is limited. METHODS Fifteen fresh frozen human cadaveric lumbar spine segments ...

Comparison of outcomes and safety of using hydroxyapatite granules as a substitute for autograft in cervical cages for anterior cervical discectomy and interbody fusion

Background:   After cervical discectomy, autogenetic bone is packed into the cage to increase the rate of union between adjacent vertebral bodies, but donor site–related complications can still occur. In this study we evaluate the use   of hydroxyapatite granules as a substitute for autograft for interbody fusion.     Methods:   From November 2008 to November 2011, 236 patients participated in ...

Lateral Lumbar Interbody Fusion

The lateral lumbar interbody fusion (LLIF) is a relatively new technique that allows the surgeon to access the intervertebral space from a direct lateral approach either anterior to or through the psoas muscle. This approach provides an alternative to anterior lumbar interbody fusion with instrumentation, posterior lumbar interbody fusion, and transforaminal lumbar interbody fusion for anterior...
